Requirements

Child:

• Original state birth certificate

Parents:

• Should be registered, active members of Assumption Church or another Catholic Church
• If the parents are not members of our parish, they should have a connection to Assumption Parish through family members who are parishioners or reside within the parish’s territory. In addition, parents must provide a letter of permission from their pastor authorizing the baptism of their child outside their home parish.
  • If you are not a registered member of a Catholic Church, you must request this letter of permission from your local Catholic Church
• We must have approval from both parents allowing the child to be baptized

Godparents/Sponsors:

• Godparents must be baptized Catholics, 16 years old and older, and have received the sacrament of Baptism, Eucharist and Confirmation. If Godparents are married, must be married in the Catholic Church. Only two godparents are permitted.
• At least one godparent is required. The godparent must be a baptized Catholic who meets all of the above requirements.  
• Per Canon Law, a second godparent may also be chosen if they meet the same requirements. If someone wishes to participate but does not meet the requirements, they may serve as a witness instead.
